I stumbled across this app while looking someones posts. What do you do in this app ?

You write programs and do stuff. If you want to invent your own 3ds games to play, then this app is great!

along with Aiden's comment you can also share your games through Smile Basic with other Smile Basic users to play. But you can also code other programs too.
